AMH Department
Geriatric Care
Patient-centred support for older adults, with attention to function, mobility, current treatment and individual care goals.
About the department
Care begins with an individual assessment
The geriatric care department takes a whole-person view of the older adult's concerns, medicines, mobility, daily activities and support needs. Care planning should involve the patient and, with permission, family or caregivers.
